You can't improve what you don't measure. It's a principle that's driven engineering, manufacturing, and business for decades. Yet in mental health and wellness practice, many clinicians still rely on clinical intuition alone to gauge whether their clients are truly getting better.

That gap is closing. Measurement-based care (MBC) has moved from the margins of evidence-based practice into the mainstream, driven by regulatory requirements, digital health infrastructure, and a growing body of evidence showing that regular outcome tracking improves client outcomes significantly. But knowing MBC works and knowing how to actually implement it in your practice are two different things.

In 2025, CARF (Commission on Accreditation of Rehabilitation Facilities) updated Standard 2.A.12 to require documented, written MBC procedures for any organisation providing mental health or rehabilitation services. G0553 reimburses digital mental health treatment at $20.06 per 20-minute session; G0554 provides $19.73 for each additional 20-minute block. In November 2025, CMS further expanded coverage to include ADHD digital therapeutics, effective in 2026.

This is gold-standard feedback. If a client rates the SRS at 30/40 consistently, you have direct evidence that something about the alliance needs attention.

CORE-OM: When You Need More Detail

For more comprehensive tracking, particularly in clinical settings serving populations with complex presentations, the CORE-OM (Clinical Outcomes in Routine Evaluation, Outcome Measure) offers a 34-item assessment of general psychological distress, trauma, physical problems, and risk. It's longer than PHQ-9 or GAD-7, so it's less suited to every-session administration, but it's powerful for initial and periodic review (every 4-8 sessions).

WHO-5: Wellbeing Rather Than Illness

The five-item WHO Wellbeing Index (WHO-5) flips the frame. Rather than measuring symptom severity or distress, it measures positive functioning: feeling cheerful, calm, active, full of energy, waking refreshed. Interpreting Your Data: What the Numbers Actually Tell You

Collecting measures is one thing. Making sense of them is another.

The basic principle is simple: you're looking for trajectories, not snapshots. A single PHQ-9 score of 14 tells you something. But a PHQ-9 score of 23 dropping to 18 to 14 over three sessions tells you much more. You're tracking velocity, not position.

With PHQ-9 and GAD-7, your clinical eye should focus on two thresholds. The first is the standard cutpoint for clinical significance (10 for PHQ-9, 10 for GAD-7). The second is reliable change: researchers typically define this as a change of 5 points or more on PHQ-9 or GAD-7 as a meaningful shift in a single client's trajectory. The SRS is perhaps the most direct: below 36/40 on the alliance measure is a signal that something in the relationship needs to be named and worked with.
